ISS Forms & Guides

ISS compiled a list of forms and resources international students will need while studying in the United States (US). Please email ISS for assistance if you cannot locate the needed form or resource.
Why are forms and signatures required?
There are times when managing or making changes to your academic record or student account requires interactions from a specific department, office, staff, or faculty. When facilitating these interactions, forms are required to ensure policies are understood, approvals or recommendations are obtained, expectations are met, and documentation is recorded.
A signature can represent approval given for a transaction and means a conversation took place and two or more parties reached an understanding. If a form requires a signature from a specific person or department, a meaningful conversation must occur before action is taken to comply with policies, guidelines, or regulations.
Can I use a digital (electronic) signature?
Most forms are set up to allow digital signatures. However, some forms may require a physical signature from the student (i.e., Form I-20). If a physical signature is required, it will be noted, and the form will not be formatted to accept a digital signature.
If you are interested in using a digital signature on forms at Florida Poly and do not have one set up, please view this document for assistance.
Note: Forms are listed alphabetically under each category.
